About you?

You will need to be flexible as the shifts ranges from morning, afternoon as well as respite care.

You will be able to work with individuals from a range of social, financial, and cultural backgrounds and have excellent communication skills. You will have the ability to work unsupervised and are able to follow the duties and tasks of a designated support plan. You will be able to conduct timely and accurate record keeping and objective report writing. You will have a current WA ‘C’ driver’s license and are able to travel as needed within the Greater Perth Metropolitan area. You will demonstrate a can-do attitude, like a challenge, are flexible, value people, choice, culture, and diversity.

The job - What you’ll be doing

You will be responsible for delivering a broad range of high-quality and flexible support to people with disability, their families, and carers to develop skills and capacity to promote independence in their home and community. You will be supervised by the Neighbourhood Team Leader. You will assist customers with personal care, daily living tasks and community inclusion, whilst ensuring all possible plan outcomes and goals are delivered and met.

Your responsibilities:

  • Deliver person centered support to customers that is reliable, timely and flexible involving daily activities and community access, recreational activities, social support, transport, providing respite care and other requirements specified on the support plan.
  • Build strong relationships with customers, their families, and key stakeholders.
  • Assist customers discover activities they enjoy that will boost their social interaction and inclusion.
  • Encourage growth, development, and independence of customers through role modelling and mentoring.
  • Provide regular updates to key stakeholders regarding the customers you are supporting.
  • Contribute to continual improvement processes, enabling improvement within the organisation.
  •  Pro-actively works on own personal and professional development.

Key requirements:

  • Valid working rights
  • NDIS Screening Clearance
  • NDIS Worker orientation module certificate
  • Current WA Class C drivers’ licence and own reliable car

  • You will deliver high quality support to people with a disability, their families and carers that develop skills, capacity and promote independence in their home and community.
  • You will ensure all possible outcomes and goals are delivered and met for customers.
  • You will ensure reporting requirements are met for key stakeholders.
