Support Coordinator - Rockingham/Mandurah

We are looking for an experienced Support Coordinator who shares our passion with helping people with disabilities live a good life. This role is part time (3-4 days a week, 0.5 to 0.8 FTE) which means we can work with you to create a roster that is suitable for both parties. This role will be working from home only, with the support of a team within reach - just a phone call or MS Teams message away!

As a Support Coordinator, you will work in partnership with the people that we support to identify their goals and aspirations. You will help a person to achieve their goals and desired outcomes by understanding their individual needs and linking them with the appropriate services funded through their NDIS Plan.

  • Be influenced by your knowledge of a person-centered approach to develop individualised strategies and outcomes for the people we support
  • Be good at puzzles! Problem solving skills are a must to ensure that high quality, customer focus, safe and well organised services are delivered 
  • Have established connections in the community to ensure quality supports and services for our customers
  • Ability to work as a team, succeed as a team, celebrate as a team - team work makes the dream work!

To join the Interchange Team, you need to have:

  • Tertiary qualifications in disability services, community services, or social work, and proven experience in the field
  • Proven experience as a Support Coordinator
  • A demonstrated commitment to the values, vision and mission of Interchange
  • Excellent verbal and written communication skills, including report writing
  • A high level of attention to detail and accuracy
  • The ability to maintain customer confidentiality
  • Excellent time management skills, including the ability to effectively manage multiple projects and effectively set priorities
  • Current WA Driver’s Licence
